The project form
The contact form sends your practice, name, work email, phone, chosen project, software and note to a private ClearMax Labs workflow and a spreadsheet it controls. The page and time travel with it. A hidden trap field filters bots. Messages are kept so that we can reply and keep a record, and they are erased on request.
Calls to this site's number
Calls to this number may be recorded or answered by an automated assistant. Please do not share patient information by phone.
Site services
Cloudflare Pages serves the site and logs each request, including its address, page and time. Page use may be recorded by Microsoft Clarity through cookies, with form fields masked.
If something goes wrong
RCW 19.255.010 sets thirty calendar days for telling affected consumers about a breach. Notice can be delayed if law enforcement determines it would impede a criminal investigation. ClearMax Labs would contact you directly.
